NOTE: BE SURE TO USE YOUR FULL LEGAL NAME (Example: If your name is Michael, you must enter that, even if you go by "Mike")

Be sure to complete ALL required fields of information including the NAME, AGE & GENDER that you would like to coach.

• Choose Registrant, Choose Category, enter Registrant Information, (Ignore) Photos, check Waivers, Review, Receipt=Thank you for registering!

• Registration Form Information. Your registration form(s) will be emailed to you. Then *PLEASE email to: sandi.monroe@yahoo.com (If you do not have email, please contact your administrator or return to your Member Account and click the My Registrations button and the Print Form button for the current season). 

• Please make sure you check to receive our confirmation email.  IMPORTANT=If attached to the confirmation email is a request for Live Scan form please down load this form and bring with you to the Live Scan location noted below (there is no cost to you). Please complete this requirement at your earliest convenience as the process can take 2-4 weeks, or more, and you cannot be assigned to a team until approved.
• Bring your completed Live Scan form to: Truscan Finger Print Services 2324 4th Street, Santa Rosa, CA 95404 707-566-7226 or DL7 3775 Brickway Blvd., STE 110, Santa Rosa, CA 95403 707-526-5888. 
• NOTE: If you do not have a form attached to this email you are already approved= a returning coach and all you need to do is forward your confirmation (Only new coaches will receive the "Live Scan" request w/their email. If you are returning coach, thus would have already completed the Live Scan, you ONLY need to complete this sign-up, registering to coach and forward your email confirmation).
